THP Plus is a goal-based program. Once a youth is enrolled in the program, he/she will work on program goals by engaging in productivity, such as employment, academic/vocational school, budgeting and life skills education and parenting education.

Participants enrolled in the program will be housed in one of two models:

Host Home - Participants may continue to live with their foster family or another caring adult with whom they have a permanent connection. The ultimate goal for those living in a host home is for transition into permanent independent housing by program exit.

Independent Housing - Those participants who are ready for their own rental may choose to live independently or with a housemate. Participants may receive assistance with locating and moving into their permanent housing.

Participants enrolled in the program will receive the following continuum of services:

  • Case Management Services
  • 24-hour Crisis Intervention
  • Educational Advocacy and Support
  • Job Readiness Training and Support
  • Mentoring
  • Rental Subsidies
  • Utility Assistance
  • Grocery Allowance
  • Aftercare Services

The maximum length of enrollment in THP Plus is 24-months. The program is voluntary and intended for young adults who have the capacity to safely reside in unsupervised settings.
